2. APPLICATION PAPERWORK

Following the receipt and review of the pre-credential questionnaire, the official credential application and corresponding paperwork will be emailed to you (or mailed upon request). You will send this completed application, including a photo, notarized release forms, copy of Social Security card, and application fee to Debbie Eason, deason@simn.ag.   

*Please Note: There are additional steps to be taken, should the applicant have a previous divorce.
